For each employee you must have a federal W-4 and state withholding form on file. These forms advise you of the exemptions the employee is claiming for payroll tax withholding purposes. 

Please use the links below, print the necessary forms and have the employee sign and return to you.

You must keep these forms on file for future reference.

At year-end all employees are issued a form W2 and State Form. Please contact our office to either prepare these forms for you or assist you in preparing them from your location.

For most states you will need to look at the applicable state's Department of Labor website to obtain an unemployment employer number.
